Information We Collect
When you reserve a slot we collect your name, age, mobile number, email address, and the health interests you select. During consultation we may collect medical history, diagnostic results, and treatment records.
How We Use Your Information
We use your information to contact you about your reservation, schedule your consultation, prepare your treatment plan, and send event and clinical updates you have agreed to receive.
Medical Confidentiality
Health information is treated as sensitive personal information under the Data Privacy Act of 2012 (RA 10173). Access is limited to your attending physicians and the concierge team supporting your care.
Sharing and Disclosure
We do not sell your data. We share it only with our medical staff, accredited laboratories performing your diagnostics, and service providers bound by confidentiality agreements, or where required by law.
Data Retention and Security
Records are retained for the period required by Philippine medical record-keeping rules and stored with encryption and access controls. Enquiry data not converted to a patient record is deleted within 24 months.
Your Rights
You may request access, correction, deletion, or portability of your personal data, and may object to marketing at any time by emailing info@revivee.ph.
